Oilp is actually what I'm using. It runs without extra input lag most of the time but there are still occasional lag spikes. They're bad enough to end a run if thay happen in a difficult pattern. They happen each run but are random and I can't figure out what triggers them. They happen with no patch, vpatch, and oilp, so it's likely something besides the patches. I don't recall these lag spikes on my previous computer. It happens maybe 3 times per run but always at different times. I think it may have something to do with running the game on modern hardware. Messing with my video driver settings doesn't help. I recently played UFO on a friend's computer and didn't notice any lag spikes. I've gotten LNNN on this exact setup, so it's not enough to make playing impossible, it's just a nuisance.
In the early days, I faced even more annoying bugs, such as when an arrow key got stuck and stayed activated even when I stopped pressing the key. It required pausing the game, hitting the key until it stopped being activated, then continue playing. The fix was to use a USB to PS/2 adapter. Those would break a lot and I'd have to keep replacing them. This is no longer necessary with my current hardware. Keys never get stuck even with USB.
It might be a good idea to get a new keyboard. The last time I got LNNN, it was soon after getting a new one. When I played on my friend's keyboard, it felt nicer, even though we both use the same switch type. I'd like to get one where the switches are replaceable without soldering. Previously, I tried learning how to play with 4 keys in a straight line, like a piano, so that I don't have to move my middle finger between the up and down keys. It didn't work well and I'm sticking with the normal arrow keys.

After putting hundreds of hours into a shmup that has a high FPS, Touhou just doesn't feel right. The input feels rough and it feels like I can see objects in motion skipping. The game feels like a slideshow. Low FPS works for things like anime, or turn based games, but it's not for shmups. Playing shmups with input lag is AIDS. Touhou is slow enough that it's possible to go through small bullets when unfocused. This is demonstrated the easiest with ReimuC in SA.

There's ways to debloat Windows 10. Disable animations in the desktop. Disable vsync in the driver settings. Disable anything unnecessary in msconfig.

Maybe this is a good time to get better at certain things besides just playing. Analyze replays. Decompile the ECLs and learn how the patterns work in code.
